Authorities from the area said 32-year-old Joshua Fornash was killed and 22-year-old Monica Chavez was injured when the motorcycle they were on allegedly collided with a car that was turning into a parking lot along West 16th Street. Paramedics pronounced Fornash dead a short time after the accident, while Chavez was rushed to the hospital with serious injuries stemming from the accident.
