Details:
Kristen Ady was killed and John Ady and Rhonda Dam injured in a four vehicle chain-reaction accident on Route 422 in Slippery Rock Township, PA. Pennsylvania State Police say the initial crash occurred around 3:57 p.m. on Friday, April 11, 2025.
A commercial pickup driven by Zachariah Kyle reported set off the collision when it collided with the rear of the Hyundai driven by John Ady. It rolled onto the driver side of the vehicle, striking a Smart Fortwo driven by Dam, which then collided with a Honda Accord.
Kristen Ady was pronounced dead at the scene, and John Ady was airlifted to St. Elizabeth’s Youngstown Hospital with serious injuries. Authorities are currently investigating what caused the initial collision.
